Dear Patient:
Welcome to Atlantic Surgery Center and thank you for allowing us to care for you during your surgery. Our goal is to treat each patient with respect and dignity. We deliver state of the art ambulatory surgical services in a caring and compassionate environment. Our primary concern is your health and safety.
Atlantic Surgery Center is a multi-specialty center providing care for many types of surgical needs. Most days 3-4 surgeons are working simultaneously and your physician’s line up may not move as fast as others, so you may see patient checking in after you that are called prior to you.
In the links below, you will find a brief health history form with an area to list all your current medications and herbals with dosages. Please complete the form and have it available for the pre-admission phone call you will receive from the center. Bring the form with you on the day of surgery, if possible. There is also a map and directions to the surgery center. Atlantic Surgery Center does not provide emergency care. In case of an emergency always call 911 or go to the nearest emergency facility. You will also find a detailed list of Patient Rights and Responsibilities, Ownership Disclosure, Privacy Policy Notice, and billing policies. There is a complete list of contact information for feedback on our services for your convenience. All this information and more is listed on this web site. The Center respects and upholds the right of a patient to participate in health care decisions and to execute Advance Directives. An Advance Directive is a legal document that guides significant others and health care providers to follow a patient’s wishes should the patient become incapacitated or unable to make decisions. It is the policy of this Center, due to the nature of the outpatient procedures performed, to always attempt to resuscitate a patient and transfer that patient to an acute care facility regardless of any Advance Directive. Since you will have anesthesia during your surgery, you will not be allowed to drive yourself home. Please bring a responsible adult with you to drive. If you do not have a ride home, your surgery will be cancelled. Your physician will discuss your procedure and confirm you understand the risk of your scheduled procedure, alternatives to your procedure, and possible complications prior to signing your informed consent. Once you are awake, our staff will review your discharge instructions with you and your care giver. You will also receive a patient satisfaction survey to let us know how well we provided services.
